1. Service Overview
Blue Torch Home Care Limited (“Blue Torch”) provides professional home healthcare services, including nursing care, dementia care, caregiver services, travel nursing, hospital aide support, and phone consultations.
Our services are delivered based on individual assessments and customized care plans designed for each client.
2. Service Commencement
- Services will begin after an initial home assessment (where applicable) and confirmation of payment for the required service.
- A personalized care plan will be developed based on the assessment findings and client/family preferences.
3. Client Responsibilities
- The client or authorized family representative must provide accurate information about the client’s health status, medical history, medications, allergies, and any special care needs.
- The client or family must ensure that the home environment is safe, clean, and suitable for caregiving activities.
4. Limitations of Liability
- Blue Torch Home Care Limited will provide skilled and professional caregivers, nurses, and/or doctors based on service needs.
- However, Blue Torch shall not be liable for:
- Any accidents, injuries, sudden health deterioration, deaths, losses, or property damages occurring in the client’s home or premises during care.
- Incidents arising from pre-existing health conditions, natural causes, environmental hazards, or third-party actions beyond the reasonable control of Blue Torch staff.
- By subscribing to our services, the client agrees to accept these risks inherent in home healthcare delivery.
5. Caregiver Conduct
- Blue Torch screens, trains, and monitors its caregivers and nurses to meet professional standards.
- Any concerns about a caregiver’s conduct must be reported immediately to Blue Torch management for investigation and action.
6. Payments
- Assessment fees, service fees, and product purchases must be paid in full before the start of service unless other arrangements are made.
- Ongoing services (e.g., monthly care) must be paid in advance for the upcoming service period.
- Wallet pre-funding options are available for clients who prefer flexible payment.
7. Cancellations and Refunds
- Clients must provide at least 48 hours’ notice for any cancellation or postponement of scheduled services.
- Refunds (where applicable) are subject to deductions for services already rendered and administrative costs.
8. Confidentiality
- All client information, medical history, and service details will be treated as strictly confidential and used only for the purpose of providing care.
9. Emergency Situations
- In case of medical emergencies, Blue Torch caregivers and nurses are authorized to provide basic first aid and contact emergency services (ambulance, hospital) as necessary.
- All costs related to emergency services will be the responsibility of the client or their family.
10. Changes to Terms
- Blue Torch Home Care Limited reserves the right to update these Terms and Conditions at any time.
- Clients will be notified of any significant changes in writing or via official communication channels.
Client Service Agreement
Monthly Payment Terms
- All care services must be prepaid within the first 3 days of each new month.
- A 48-hour grace period is granted. If payment is still not made, services may be suspended.
- If payment is not made by half of the care period (e.g. Day 16 of a 30-day plan), a ₦500 daily penalty will be added until payment is completed.
Refund Policy
- No refunds after 15 days of starting a new care cycle.
- If cancelled within 15 days, unused days will be refunded after subtracting days already serviced.
- Assessment and registration fees are non-refundable.
Zero Abuse Policy
We maintain a zero-tolerance policy for all forms of abuse toward our staff, including:
- Verbal abuse
- Physical violence
- Sexual harassment
- Emotional or financial abuse
Any abuse will lead to immediate termination of services and legal action.
Client Responsibility
- Provide a safe and clean home environment.
- Pay service fees as agreed.
- Treat Blue Torch staff with dignity and respect.
Agreement
By subscribing to Blue Torch Home Care Limited services, the client (or authorized representative) confirms that they have read, understood, and agreed to these Terms and Conditions.
